So once call setup has been established...the voice traffic would be redirected (direct connect between VOIP clients) and go via the internal business network where hopefully QOS has been implemented....Or am I wrong Isnt it just the call manager in the cloud..

VoIP means Voice over IP but does not necessarily mean over the Internet. There is no quality of service in this model.
